    Conduct disorder (often called juvenile delinquency or the Antisocial Personalisty Disorder of Childhood) is a psychological condition of childhood and adolescence characterized by a pattern of repetitive disruptive behavior wherein the rights of others or social norms are intentionally violated.  Said another way, children and adolescents with conduct disorder have difficulty following rules and behaving in a socially acceptable manner.       

    Symptoms of conduct disorder may include:

    • Cruelty to people
    • Cruelty to animals
    • Stealing
    • Running away
    • Lying
    • Fire-setting
    • Skipping school
    • Breaking and entering
    • Property destruction
    • Forcing sexual activity on others
    • The use of weapons in fights
    • Frequent physical fights
    • Alcohol or drug usage
    • Cheating
    • Manipulating
    • Physically or verbally bullying
    • Intimidating others
    • Outbursts
    • Impairment in social, school or occupational functioning
    • Violating curfews
    • Rule breaking

     

    Conduct disorder affects 1 to 4 percent of 9- to 17-year-olds, depending on exactly how the disorder is defined. 

    Adolescents with more severe Conduct Disorder (characterized by hospitalization or day treatment) that do NOT receive early and appropriate treatment are more likely to have relationship and occupational problems in adulthood. They may engage in illegal or criminal activity or exhibit other antisocial behaviors. These behaviors include chemical dependency, school failure, unwanted pregnancies, injuries, or suicidal tendencies.

    People with Conduct disorder are also at greatly increased risk for incarceration, injury, depression, substance abuse, and death by homicide and suicide.

    Among adolescents suffering from Conduct disorder ("impulse disorder"), the risk of suicidal behavior is even stronger in developing countries (red bars).



     

    No matter the type of country, the risk is considerably increased by co-morbid alcohol and drug use (both highly common in people with Conduct disorder).

    After the age of 18, up to 40% of children with conduct disorder grow into adults with Antisocial personality disorder [Antisocial personality disorder behavior consists of two components: the presence of antisocial (i.e., angry, aggressive, or disobediant) behavior and the absence of prosocial (i.e., communicative, affirming, or cooperative) behavior]. A universal characteristic of antisocial children and adolescents is that they appear to have no feelings. People with Antisocial personality disorder demonstrate NO care for others' feelings or remorse for hurting others. People with Antisocial personality disorder tend NOT to show their feelings except for anger and hostility, and even these are communicated through aggressive acts and are NOT usually expressed through affect. 

    Research shows that youth with aggressive conduct disorder have abnormally increased activation (as demonstrated by increased blood circulation) in specific areas of the brain when confronted with empathy-eliciting stimuli. Researchers compared 16- to 18-year-old boys who were diagnosed with aggressive conduct disorder to a control group of adolescent boys who did NOT meet the criteria for the diagnosis. The conduct disorder group showed very strong activation of the brain regions called the amygdala and ventral striatum when watching pain inflicted on others (see the MRI pictures below). These areas of the brain respond to feeling rewarded; so this abnormal activation suggests that the Conduct disorder group enjoyed watching pain. Also, the Conduct disorder group did NOT activate the paracingulate cortex and temporoparietal junction (areas of the brain involved in social interaction and moral reasoning).

    When people with aggressive conduct disorder watch an individual intentionally hurting another (closing a piano lid on someone's hands), regions of the brain involved in reward processing are activated (the amygdala and ventral striatum).

    Twin studies also can be used to assess the extent to which the co-occurrence of disorders is influenced by genetic and/or environmental factors. Researchers studying "externalizing behaviors" in twins found that if one twin has Conduct disorder (or another externalizing disorder), then the chance of having the exact same disorder is higher if the twins are identical (i.e., monozygotic) twins, who share 100 percent of their genes, than for fraternal (i.e., dizygotic) twins, who share on average only 50 percent of their genes. In other words, genes influence the risk of conduct disorder and adult antisocial behavior (as well as alcohol and drug dependence).

    Since this correlation was true for all the "externalizing disorders", the researchers concluded that a significant portion of the genetic influence on the expression of conduct disorder and adult antisocial behavior is through a shared genetic mechanism; the same genetic mechanism that leads to all of the "externalizing psychopathology disorders." In other words, if one has the genes for "externalizing disorders", then that genetic influence may produce any of the externalizing disorders (i.e. as alcohol or drug dependence or Conduct disorder/Antisocial personality disorder). Said another way, all of the externalizing disorders are related and manifestations of the same genetic mechanism.

    Electrophysiological studies are recordings of the brain’s electrical activity. These studies, which are conducted using electrodes placed on the person’s scalp, provide a noninvasive, sensitive method of measuring brain function in humans. They generate a predictable pattern in the height (i.e., amplitude) and rate (i.e., frequency) of brain waves that can show characteristic abnormalities in people with certain types of brain dysfunction. For example, electrophysiological abnormalities have been observed in people with a variety of "externalizing disorders" as well as in children of these people. These findings suggest that electrophysiological measurements can be used as markers of a genetic vulnerability to "externalizing disorders."

    Attention deficit hyperactivity disorder (ADHD) is frequently co-morbid with Conduct disorder, and between 20% and 40% of children diagnosed with ADHD develop Conduct disorder; however, ADHD without Conduct disorder may be a distinct disorders from ADHD with Conduct disorder. Also, comorbidity with Conduct disorder predicts very early onset  of drug and alcohol abuse in both ADHD and non-ADHD individuals.
